Our Practice
All of our physicians have privileges at Piedmont Hospital.
GYN patients aged 40 and better can now schedule screening mammograms and bone density scans directly with our front desk or schedulers. The AWHS Imaging Center is conveniently located just across the hall in Suite 280.
Our office hours are 8:00am to 4:00pm Monday – Friday. There is always a physician on-call after business hours and on weekends for emergencies only.
We do not accept walk-in appointments or same day new patient appointments. If you are an established patient and would like to be seen on the same day for a problem visit you may call the office and ask to speak with your physician’s Medical Assistant. Each call will be considered and triaged appropriately.
Office Policies
You must cancel at least 24 hours before your scheduled visit, or you will be charged a $50 no-show fee.
We allow a twenty (20) minute grace period. If you arrive more than 20 minutes late for your appointment, it will be at the physician’s discretion whether you will still be seen or will have to reschedule.
Please bring your insurance card, completed forms, and your copay (if applicable). Please bring any previous medical records pertinent to your visit.
If you must fill out your forms in the office, we ask that you arrive 15 minutes before your appointment time to ensure ample time for completion. In an effort to maintain accurate records, we ask that you complete a demographic sheet each year.
We are located in the Piedmont Physicians Plaza at 275 Collier Road NW. You may enter the 275 Building parking deck from Collier Road. Once you have parked, take the parking elevator to Level P7. On P-7, you will take the elevators (to the left) to our office, Suite 230, on the 2nd floor.
Unfortunately, we do not validate parking. Parking deck rates are: 0 to 30 minutes – Free, 31 minutes to 4 Hours – $7.00, 4 hours or greater (up to 24 hours) – $10.00. The electronic pay stations accept cash or credit card (no Apple Pay).
We love seeing your babies and understand that patients often bring them to appointments. However, for the safety of all patients, children, and staff, we strongly prefer that sick children be kept at home and kindly ask that children not be brought into the lab area where blood is being drawn.
You may request your medical records by completing a Medical Records Request Form. This form is available online, or you may pick one up at the Front Desk during your visit. Once the form is completed and signed, you may return it to our office via fax, mail, or email at [email protected]. Please allow at least 72 hours from the time we receive your request for the records to be sent.
NOTE: There is no charge for records requested for continuity of care (i.e., when records are sent directly to another healthcare provider). However, fees will apply for personal copies, attorney/legal requests, insurance company requests, and workers’ compensation requests.
Prescription refills can be submitted via fax by your pharmacy, requested by calling your physician’s Medical Assistant, or sent as a MyChart message. Please allow 24 hours for processing (pending approval).
GYN/Annual Visit
Please call to schedule your annual appointment 6-8 weeks prior to the date you are due to return for your yearly visit.
Your yearly exam is a preventive visit during which your provider will conduct a general physical exam. This includes a breast exam, pelvic exam, and a pap smear if needed. This is also an opportunity for you to update any changes to your personal history, family health history, and current medications. Your physician will evaluate the need for any health screening tests or immunizations based upon your age and history. This is also the time to request medication refills.
Please keep in mind that the annual exam does not include the discussion of ongoing chronic conditions or new concerns. You must schedule a separate appointment problem visits.
STD testing is not included in the annual exam but can be performed at your request. You will be asked to complete the optional STD information and consent form. This form also lists the cost of each test and the CPT codes so that you may check your insurance benefits prior to screening.
You will typically receive your test results through a combination of methods, including phone call, MyChart portal, or mail. If results are abnormal, your provider or a Medical Assistant will likely call you to discuss them and schedule any necessary follow-up.
NOTE: You may see results in MyChart before your healthcare provider has had a chance to review them. If you have questions or concerns about your results, it’s always best to contact your provider’s office directly for clarification and discussion.
OB Visit
It is best to schedule your pregnancy initiation between 2 to 4 weeks after your missed period.
Typically your pregnancy initiation appointment does not include an ultrasound. However, it is at the discretion of the physician to recommend an ultrasound at that time.
Our ultrasound rooms are not very large. For this reason, you may bring only one (1) guest with you into the ultrasound room. Our sonographers must be able to easily access the equipment and efficiently scan the patient.
Yes, we do. If you have already received obstetric care but are interested in transferring to our practice, please call our office at (404) 352-3616 and request to speak with our OB Office regarding your possible transfer. A brief history will be taken over the phone, and you will be required to fax your medical records for physician review. Once your records are received and insurance benefits are verified, a member of our staff will contact you with the physician’s decision.
NOTE: If you have been dismissed from any other practice in the AWHS group, we will not be able to accept you as an OB transfer.
No, we do not have midwives.
We currently offer three classes for our established patients: Child Birthing, Baby Basics, and Breastfeeding. You can sign up by contacting our OB Office.
Insurance & Billing
We accept many different insurance carriers. Please visit our Billing & Insurance page to view the list.
It is your responsibility to ensure that we have valid insurance information on file for you. You must present your insurance card at check-in for each visit. We may ask you to reschedule your appointment, or pay cash for services provided, if you are unable to present your insurance card upon check-in.

If you do not have health insurance, or are unable to produce the appropriate documentation to verify coverage, you will be a “self-pay” patient.
Payment is required upon check-in. Charges will be determined based upon customary coding and billing practices and will reflect the nature of your visit and the services received. You will receive an additional bill from a separate lab facility for any lab work that is performed.
All Self-pay patients receive a 20% discount when payment is made in full on the same day that services were provided.
Payment of the patient responsibility portion of your visit is due at the time of service, including all applicable co-payments. For your convenience, we accept cash, checks, Visa, MasterCard, and American Express. All returned checks are subject to a $30.00 handling fee.
